Criminal Background Check conducted through Florida Department of Law Enforcement
FBI Criminal Background Check (for Caregivers under specific programs)
Current License or Certificate in good standing
Affidavit of Good Moral Character
Verifiable work references
Employment Physical and TB test indicating employee is "free of signs and symptoms of communicable disease". Click here
